Injections of corticoid preparations can have severe side effects. In this issue of Deutsches rzteblatt International, Christian Holland and coauthors contribute to physicians' awareness of problems of this type with a report on the relevant findings of medicolegal expert committees in Germany (Dtsch Arztebl Int 2012; 109[24]: 425-30.

One patient, for example, received multiple intramuscular injections of dexamethasone and diclofenac for the treatment of back pain. Six weeks after the last injection, 500 g of necrotic tissue had to be surgically removed from the site of the injections; a subsequent wound infection led to multiple further hospitalizations. The authors describe both aseptic and septic complications, including abscesses and purulent joint infections. When they affect the spine, such complications can cause weakness of varying degrees of severity, ranging all the way to para- or tetraplegia. Fatal sepsis can also occur.

From 2005 to 2009, the German medicolegal expert committees and arbitration panels dealt with 278 cases of complications after corticoid injections. Medical errors were found to have been committed in 40% of cases. Typical errors were faulty asepsis, treatment without indication, and injections that were too closely spaced in time or in excessive doses. Furthermore, whenever it is determined that a patient has not given legally valid informed consent for an injection, the physician is liable for any and all adverse consequences of the injection for the patient's health. By giving physician readers the appropriate knowledge base, the article is intended to help them avoid such difficult medicolegal situations.

As the nature of our media shifts, the nature of our communications offices will be forced to shift as well.

Recently, GovLoop member Dave Hebert asked the community: "What will the future of government communications look like?" Even though he leads internal communications at his agency, he was asked to assume responsibility for public-facing web content. Their functions are fairly different -- as Hebert describes, "One is concerned with an audience, and the other is a communications medium and construct. And trying to balance the often-disparate needs of each function was, at times, a little schizophrenic."

Is there a way to house both of these departments under one roof? With budget constraints and personnel cuts, it appears that whether we are ready or not, both traditional and digital -- and internal and external -- communication teams will have to find a way to work together.

Hebert sees the future of government communications relying on three basic competencies: writing/storytelling, digital/multimedia, and measurement. It is important to create captivating content with quick headlines that will grab attention on mobile and social media, to have a diverse set of journalism skills, and to be flexible and responsive to the audience.

Building on those competencies, Dannielle Blumenthal outlined the traits that a communication team member needs: "Must be technical. Must be versatile. Must work well under pressure. Must be resourceful. Must not worship words but welcome video, photography, design, multimedia of all kinds. Must constantly learn. Must have amazing project management skills and be a true team player. Must get coffee twice a day because the job is always two or three jobs in one."

Dan Slee also listed what press offices need to accomplish, starting with the basics:

Have basic journalism skills.
Know how the machinery of local government works.
Write a press release.
Work under speed to deadline.
Understand basic photography.
Understand sub-editing and page layouts.

As Slee says, "there's no point in having an old school team with spiral-bound notebooks and in the next room a digital team with jet packs and Apple Macbook pros not communicating."

Hello AvengerSpirit, I've been adjusting my character for quite some time now. I'm not sure what u meant by fleshed out, but I added an extremely detailed backstory to Mai'k Mai'k. Then I changed his photo thinking that fleshed out meant make him look more human and not anime, maybe you thought the picture was not human (which it was) no harm none the less. I have no problem reaching your 200 word minimum and the only reason my minimum was so low before is because I had just started. Another thing is that I can't send private messages for some odd reason or reply to them, so this is the only way I can reach you. I'm not adding anything else to my character because I think I've done more than enough. Please look over his profile again and tell me what you think. I'm really looking to join this rp, but if not it's no big deal, so don't sweat it.

Apple owns and operates the entire iPad production process, from design ideas to distribution. Apple designs the iPad?s hardware, operating system, applications, and accessories. Since Apple controls every facet of the process, each component seamlessly comes together to produce the most popular tablet on the market. While Google and Microsoft have tried to compete with the iPad, their disjointed production systems have produced products that consumers deem inferior to the IPad. However, both Google and Microsoft have altered their production strategies.

The New Google and Microsoft Tablet Paradigm

Google and Microsoft have introduced measures that change their disjointed tablet production processes into a process that both companies control from start to finish. Both companies have taken control of the most essential element-hardware-rather than allow third party vendors to produce hardware components. The result has been the recent introduction of Microsoft?s Surface tablets, of which one version runs on Windows 8, while the second version runs on Windows RT. Microsoft designed, produced, and assembled both Surface tablets, instead of relying on their traditional hardware partners, such as Dell and Hewlett-Packard.

During its I/O conference beginning on June 25, Google will introduce its new tablet. While the company has offered the premium Nexus line of tablets, other companies have always produced the hardware components. HTC manufactured the first Nexus tablet, followed by Samsung, which has produced the last two versions. Now, Google will produce a tablet that mirrors the iPad and Surface business models. Google will control every facet of the product process and sell the Android tablet on its Google Play Store.

The business model changes implemented for the new Google and Microsoft tablets will have lasting implications for the iPad. While the iPad may become the generic word that describes tablets, Google and Microsoft will inevitable erode some of Apple?s huge table market share. The question technology analysts pose id just how much market share Apple will lose. The answer to that question will hinge on how Google and Microsoft price their new tablets.

The Google Android Strategy

Sources cited by DigiTimes claim Google plans to market nearly 600,000 Android tablets sometime in July. An additional 1.4 million Android tablets should be ready for sale by the end of 2012. Google has not finalized a price for the tablet, which may be how the company differentiates its tablet from the iPad. Initially, Google settled on a price of $249 for the Android tablet, but DigiTimes reports the Android tablet may go for as low as $199. Possible hardware manufacturer partners include Asus and Motorola, which Google is in the process of acquiring. Asus gets the first shot of co-branding the Android tablet with Google.

In addition to offering its tablet at one of the lowest prices in the market, Google also will leverage its vast resources by selling the Android from its own online store. Google?s decision to focus on Wi-Fi as the only way to connect Android tablets to the Internet helps keep costs low, thus allowing Google to undercut competitor pricing strategies. If Google?s Android has a display that pleases consumers and it possesses a long battery life, the new tablet should give the iPad a run for its money.

Many caregivers fail to give epinephrine when needed

Young children with allergies to milk and egg experience an unexpectedly high number of reactions to these and other foods, according to researchers at National Jewish Health. More than 70 percent of preschool children with documented or suspected food allergies suffered a significant reaction during the three-year period. Researchers also found that caregivers failed to administer the medication epinephrine in 70 percent of the severe and potentially life-threatening reactions. The study, conducted by the NIH-funded Consortium of Food Allergy Research, is published in the June 25, 2012, issue of the journal Pediatrics.

"Our findings clearly point to a need for parents and other caregivers to be even more vigilant in avoiding allergenic foods and treating reactions appropriately," said David Fleischer, MD, lead author and assistant professor of pediatrics at National Jewish Health. "They also suggest several strategies that both caregivers and healthcare workers can pursue to make mealtime safe for food-allergic children."

The study followed 512 children ages 3-15 months for an average of three years, documenting all allergic reactions to food. Over the three-year period, the children experienced 1,171 allergic reactions to food. Of the 512 children enrolled, 145 (28 percent) had no allergic reactions, 98 (19 percent) had one reaction and 269 (53 percent) had more than one reaction.

Just over 11 percent (134) of the reactions were categorized as severe, and included symptoms such as swelling in the throat, difficulty breathing, a sudden drop in blood pressure, dizziness or fainting. Almost all of the severe reactions were caused by ingestion of the allergen rather than inhalation or skin contact.

Only 30 percent of the severe reactions were treated with epinephrine, a medication that caregivers can administer to reduce symptoms while waiting for medical care. Reasons for the under treatment included failure to recognize severity of the reaction, not having epinephrine (EpiPen) available, and fears about epinephrine administration.

"It is very important for caregivers of food-allergic children to carry an EpiPen with them at all times, know how to recognize a serious reaction and how to use an EpiPen," said Dan Atkins, MD, co-author and professor of pediatrics at National Jewish Health. "Correctly using an EpiPen at the right time can save a life."

The vast majority of the reactions (89 percent) were caused by accidental exposure, attributed primarily to unintentional ingestion, label-reading errors and cross-contamination. Approximately half of the allergenic foods were provided by persons other than parents. Surprisingly, 11 percent of the reactions followed purposeful exposures to these foods. Researchers are exploring possible reasons for these intentional exposures. They speculate that it could reflect parents' at-home tests to determine if children have outgrown the food allergy.

The researchers identified several areas for improved education, including the need for constant vigilance, accurate label reading, avoidance of non-accidental exposure, prevention of cross-contamination, appropriate epinephrine administration, and education of all caretakers.
